Disability and Leprosy Pension Pension Scheme Eligibility
Age: Age must be minimum 18 years and maximum 150 years for disable person and minimum 1 year to maximum 150 years for Leprosy pension applicant.
Disability Percentage : Disability percentage must be minimum 40 and maximum 100
Leprosy Percentage : Leprosy percentage must be minimum 1 and maximum 100

In this section applicant has to fill, district, area(rural/urban),Tehsil, Name of the applicant, Gender, Date of Birth, Father/Husband name, Category (General/Minority/OBC/SC/ST), mobile number and complete address.

Bank statement : Applicant should enter his bank name, bank branch name, account number and IFSC code
Statement of Income Details: Income application number and income certificate number provided by the tehsildar

Details of Disability(For disability pension): The applicant must provide the type of disability , percentage of disability , the disability certificate number, and the issue date.
Upload the document : Upload the required documents like dob certificate, passport size color photograph, disability certificate (for handicap pension), Husband Death certificate (for widow pension)
